Submit a Customer Service Request

Request Lakewood is the city’s customer service hub. Please evaluate this request to determine if the request is asking for public records. If the request is asking for public records, please submit the request online for the City Clerk’s Office.

  1. Submit a concern or ask a question by selecting a topic and filling out the details.
  2. Your request will be reviewed by the appropriate department, and work is assigned to appropriate staff member.
  3. We strive to provide a response within three to five business days, though additional time may be needed.

Request Lakewood is not monitored 24/7. Call 911 for emergencies or for crimes in progress.

You may still submit a concern anonymously after creating an account by selecting the option to opt out of receiving city staff notifications when filling out your request. Please note that if a request does not provide accurate identifying information, it may not be able to be processed, and there will be no way for staff to communicate with the requestor.
